The purpose of the Social Worker II position in the Centralized Intake unit of Aging and Adult Services, is to provide the Intake, Education, and Referral function for the Coordinated Entry into the Homeless Services System, Adult Protective Services(APS), Guardianship, Home Centered Care, Special Assistance-In Home, Group Care Monitoring Services,  Non-Emergency Medicaid Transportation, and Emergency Assistance Crisis programs. This position will be responsible for completing the initial evaluations, provide education and, if necessary, referrals to community agencies to provide support to referents or family members whenever possible. 


This position requires the Intake Social Worker to provide direct intensive casework services to individuals and families in crisis and requires an assessment of each individual situation (housing, financial, medical, etc.) to identify solutions and potential resources available to resolve crisis.  Refer and/or connect clients with additional resources (within DSS or outside organizations or agencies) as appropriate. Provides support, education and counseling to families; the counseling function is closely linked to assessment and service implementation.  The Intake Social Worker may need to utilize a number of intervention modalities, including Crisis Intervention. 




  • Extensive knowledge of community and service programs throughout the Durham metropolitan area. 
  • Effective listening and communication skills and knowledge of the dynamics of interpersonal relationships and human behavior in a crisis. 
  • Case management, community experience, skills in crisis situations. 
  • Ability to integrate, coordinate assessment service plans
  • Knowledge of social work principles, techniques, and practices and their application to individual casework, group work, and community problems.



Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
