GENERAL DESCRIPTION:

 

Develop individualized plan of care using the Standardized Plan provided by Community Care of North Carolina for women and children. Perform other duties as assigned by supervisor, program manager, division director or department director.

 

 

 

DUTIES AND RESPONSIBILITIES:

 

  • Assist patients in locating a physician and resources for mother and baby.
  • Assist in scheduling/rescheduling appointments.
  • Provide and review educational material on pregnancy, parenting, well-baby care, and birth control.
  • Assume the role as a liaison with the physicians, clinics, and community agencies.
  • Review warning signs and symptoms of preterm labor with the patient at each encounter.
  • Performs family and child developmental assessments as needed.
  • Assist with referrals to community partners for any ongoing needs during the postpartum period.
  • Establishes and maintains contact with area medical homes.
  • Follows up by phone with frequent users of the emergency departments and with high-cost users of medical care (Medicaid patients).
  • Makes phone contacts and/or home visits to families in the program.
  • Monitors outcomes of interventions for families.
  • Maintains documentation in the state Virtual Health medical electronics system.
  • Review caseload for pending task and alerts.
  • Review reports from Care Impact (Current Pregnancies Patient List, Current Opportunities Patient List, CMHRP Current Caseload reports).
  • Participate in audit of electronic records as requested (Agency/Prepaid Health Plan Audits included).

 

 

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems.
  • Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment.
  • Knowledge of governmental and private organizations and community resources.
  • Knowledge of laws, regulations and policies which govern the program.
  • General knowledge of medical terminology, disease processes and their treatment is required In certain programs or settings.
  • Skill in establishing rapport with a client and applying techniques of assessing psychosocial. behavioral, and psychological aspects of client's problem.
  • Ability to establish and maintain effective working relationships with administrative superiors members of case load and their families, and with civics medical, social and religious organizations.
  • Ability to train or instruct lower-level social workers, students, or interns.
  • Ability to express Ideas clearly and concisely and to plan and execute work.

 

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:


Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
