GENERAL DESCRIPTION:

 

Durham County’s Aging and Adult Services division is seeking a dedicated and experienced professional to serve as the Special Assistance In-Home (SA-IH) Supervisor. This position plays a critical role in supporting elderly, disabled, medically fragile, and vulnerable adults, ensuring they receive the services and resources needed to safely remain in their homes and communities.

 

The SA-IH Supervisor is responsible for supervising and coordinating the delivery of SA-IH services and related processing functions. This includes direct oversight of professional social workers and administrative staff, ensuring compliance with Medicaid regulations and all applicable federal, state, and local policies. The supervisor provides leadership, guidance, and support for complex and high-risk cases, and fosters collaboration with clients, families, medical providers, and community agencies.

 

DUTIES AND RESPONSIBILITIES:

 

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary

 

Leadership & Supervision:

 

    • Directly supervise, coach, and support social workers and processing staff.
    • Oversee daily operations, establish priorities, and monitor productivity and quality.
    • Guide staff in resolving complex case management, eligibility, and service delivery issues.
    • Conduct performance evaluations and identify professional development needs.

 

Program Compliance & Quality Assurance:

 

    • Ensure consistent application of policies and procedures.
    • Monitor eligibility, documentation, service quality, client budgets, and processing accuracy.
    • Identify training needs and provide consultation on complex cases.
    • Promote effective collaboration with clients, families, guardians, medical providers, service providers, and community agencies.

 

Resource Management:

 

    • Ensure appropriate staffing, workflow, and coverage for timely service delivery.
    • Identify and utilize resources and funding sources to support clients in safely remaining in their homes and communities.

 

Community Awareness:

 

    • Foster community awareness and engagement regarding services and resources available to elderly, disabled, and vulnerable adults.
    • Build and maintain relationships with community partners and organizations to enhance service delivery and outreach.

 

 

KNOWLEDGE, SKILLS, AND ABILITIES: 

 

  • Extensive knowledge of social work principles, assessment, case management, and community-based services for elderly, disabled, and vulnerable adults.
  • Familiarity with Medicaid, SA-IH, Aging and Adult Services programs, eligibility requirements, and relevant regulations.
  • Understanding of community resources and organizations serving vulnerable populations.
  • Strong supervisory, organizational, and communication skills.
  • Ability to manage multiple priorities, exercise sound judgment, maintain confidentiality, and work effectively with diverse stakeholders.

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

Master’s degree in social work from an appropriately accredited institution and one year of directly related experience; or a bachelor’s degree in social work from an appropriately accredited institution and two years of directly related experience; or a master’s degree in a human services field from an appropriately accredited institution and two years of directly related experience; or a bachelor’s degree in a human services field from an appropriately accredited institution and three years of directly related experience; or a bachelor’s degree from an appropriately accredited institution and four years of directly related experience; or an equivalent combination of education and experience.
