RESPONSIBILITIES

 

This position is assigned to the Care Management for At-Risk Children. Services focus on facilitating services for children from birth to age 5 who are at high risk for, or have been diagnosed with, health or developmental problems. Provides information on parenting, is a liaison for developmental services, performs family and child developmental assessments as needed, and establishes and maintains contact with pediatric practices in the community. Links families of children enrolled in the program to physical, mental/behavioral health and community services as needed; collaborates with referral sources and community agencies, and provides information and encouragement to support the appropriate use of medical homes. Monitors outcomes of interventions for families; overall program goals include the reduction of mortality and morbidity in infants and young children.

 

This is a 12-month, full-time provisional position.

 

KNOWLEDGE SKILLS AND ABILITIES

 

  • Must demonstrate the ability to independently plan and coordinate work/visit schedule.
  • Requires a thorough knowledge of, and skill in, the application of principles and techniques employed in the field of in Child Health in particular, and public health in general.
  • Must have the ability to maintain effective working relationships.
  • Must have good verbal and written communication skills.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

Graduation from an accredited four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master’s in Public Health and graduation from an accredited school of professional nursing and one year of professional nursing experience.

SPECIAL REQUIREMENTS

 

Requires license to practice as Registered Nurse in NC by NC Board of Nursing; General Statutes of NC, Chapter 90, Article 9.
