GENERAL DESCRIPTION:

 

The primary purpose of this position is to function as a Women’s Health Nurse Navigator. The Nurse Navigator facilitates the access to appropriate and timely care and resources for women in the postpartum phase with the goal of reducing maternal morbidity and mortality. The Nurse Navigator achieves these outcomes through patient and staff education, community partnerships, and the development of programs, tools, and processes.

 

 

DUTIES AND RESPONSIBILITIES:

 

  • Interviews and provides counsel to patients
  • Performs telephone check-in within one week of delivery.
  • Monitors patients’ vital signs, weight, and urine results as needed
  • Instructs patients on postpartum care and warning signs and other education as needed
  • Maintains training log and documents dates of 100% of trainings attended; includes training certificates received as part of training log, 100% of the time
  • Accepts assignments related to health support in public health response to disasters and staff shortages and other duties as assigned
  • Develop relationships in the community that support mutual goals of reduced maternal morbidity and mortality and forms partnerships to help achieve desired outcomes
  • Develop educational materials for patients to help educate patients on postpartum warning signs integrating information from CDC HEAR HER campaign
  • Ensures that postpartum patients have appropriate referrals placed and facilitates access to healthcare resources to help reduce morbidity and mortality in the postpartum period
  • Responds to COVID19 as assigned
  • Perform related tasks as required

 

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Thorough knowledge of and skill in the application of nursing theory, practice, principles, and techniques employed in the field of public health, general nursing, and provision of women’s health and related program;
  • Considerable knowledge of and skill in obtaining a health history and performing physical examinations sufficient enough to determine if there is any deviation from normal;
  • Considerable knowledge of available resources and organizations and the ability to coordinate these as needed;
  • General knowledge of current social and economic problems relating to public health, including health disparities
  • Ability to plan, coordinate, and oversee the work of others;
  • Ability to deal tactfully with others and to exercise good judgement in appraising situations and make decisions;
  • Ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships;
  • Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions;
  • Adequate computer skills to allow communication, patient record documentation, and accessing of information.
     

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

At the minimum, candidates must have graduated from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and two years of professional nursing experience; or a Master’s in Public Health and graduation from a school of professional nursing and two years of professional nursing experience; or graduation from a school of professional nursing and three years of professional nursing experience including two years of Public Health nursing experience; or an equivalent combination of training and experience. 

 

 

SPECIAL REQUIREMENTS:

 

Must be licensed to practice as a registered nurse in North Carolina by the North Carolina Board of Nursing.
