GENERAL DESCRIPTION:

 

Serves as a Community Health Worker for the Durham Formerly Incarcerated Transition (FIT) Program.  Connects people being released from incarceration with needed health services, to include physical health, mental health and substance use disorder treatment.  Assists FIT clients with all aspects of re-entry to include housing, job training, legal needs, education, transportation, food insecurity and life skills.

 

 

TYPICAL TASKS:

 

  • Works individually with participants to connect them with health information and health care services
  • Assists participants to navigate health and human services.
  • Identifies and helps address participants’ health, housing, employment and other issues and barriers to these services. 
  • Identifies and connects residents with community resources that offer educational programming and other supportive services. 
  • Understands and educates participants on medication access options and services.
  • Assists in provider-patient care relationship building. 
  • Participates in community events to increase knowledge base of available community resources and programs.
  • Helps participants understand how to use resources to find health coverage or apply for assistance when uninsured.
  • Serves as Durham FIT program liaison and representative for Durham Detention Center medication assisted treatment services.
  • Conducts outreach to community resources and develops sustainable partnerships.
  • Actively participates in meetings and other communications for FIT program operations.     
  • Completes administrative duties associated with the FIT program and site of employment.
  • Completes training and other activities required for the position.
  • Service locations for the position are clinics, home visits, transition centers or other community settings. 
  • Performs related work as assigned.

 

 

KNOWLEDGE, SKILLS AND ABILITIES:

  •  
  • Requires good written and oral communication skills,
  • Ability to work well independently and with a team and
  • Basic computer/word processing skills
  • Strong interpersonal skills

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

Requires High School graduation or GED equivalent supplemented by two (2) years of experience in social work, health services, counseling or related work; or an equivalent combination of education, training and experience that provides the required knowledge, skills and abilities.
Requires demonstrated possession of knowledge, skills, and abilities as stated above through at least one year of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, or other social or rehabilitation services.

 

 

SPECIAL REQUIREMENTS:

 

  • History of incarceration and knowledge of issues and treatment related to substance use disorder.  Successful reentry into the community. 
  • Requires a valid North Carolina driver’s license with a three-year acceptable driving record.

 

 

PREFERRED REQUIREMENTS:

 

  • Certification and experience in peer support services.
  • Early mornings, evenings, and weekends work may be required.
