GENERAL DESCRIPTION:

 

This position will be part of the Women’s Health Team. This employee will participate in individual and group interventions for clients including providing evidence-based education, navigation, and care coordination services as well as assist with educational programs (individual and group).  This position will be responsible for assisting the Centering Coordinator with Centering Pregnancy. This position will be responsible for identifying healthcare needs and barriers of local communities and residents and identifying resources for meeting these needs and overcoming barriers for our Women’s Health programs and the services. This position will also participate in outreach opportunities to educate the community about all Women's Health and Public Health Services.

 

 

DUTIES AND RESPONSIBILITIES:

 

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

 

  • Participate in scheduling, coordinating, and assisting in the Centering Pregnancy program; assist in patient interventions per agency protocols that identify scope of practice for this position;
  • Identify and help address health issues, needs, and barriers to care;
  • Assist in relationship-building with Durham community partners as needed;
  • Document work related tasks in electronic medical record per policy/protocol;
  • Participate in outreach and community events as indicated;
  • Ability to educate community on all Women's Health Services;
  • Participation in Emergency Response and Preparedness tasks as assigned;
  • Perform additional related work as assigned. 

 

KNOWLEDGE, SKILLS, AND ABILITIES:

 

  • Knowledge and experience working with community organizations, neighborhood associations, and professional or student associations;
  • Knowledge of the importance of medical compliance and ability to educate clients about this topic;
  • General knowledge of Microsoft Office programs;
  • Familiarity with community resources;
  • Excellent written and oral communication skills;
  • Ability to work well independently and with a team;
  • Strong interpersonal skills;
  • Understanding of neighborhood and community values;
  • Ability to work with diverse communities.

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

Requires High School graduation or GED equivalent supplemented by two (2) years of experience in social work, health services, counseling, or related work; or an equivalent combination of education, training and experience.

 

 

SPECIAL REQUIREMENTS:

 

  • Requires demonstrated knowledge, skills, and abilities as stated above through at least one year of experience performing similar tasks in a hospital, clinic, nursing home, medical corps, or other social or rehabilitation organization.
  • Fluent in both English and Spanish.

 

 

PHYSICAL DEMANDS:

 

Performs light to medium work that involves: walking or standing virtually all of the time; exerting between 20 and 50 pounds of force on a regular and recurring basis; skill, adeptness and speed in the use of the fingers, hands, or limbs on repetitive operation of mechanical or electronic office machines within moderate tolerances or limits of accuracy.

 

 

SPECIAL CERTIFICATIONS AND LICENSES REQUIREMENTS:

 

Must have a North Carolina Driver’s License.

  • Hepatitis B vaccination/immunity – only required for DCoDPH employees with potential Bloodborne Pathogen (BBP) exposures.

 

Identified DCoDPH HCP with potential Bloodborne Pathogen (BBP) exposure have an additional requirement for the Hepatitis B vaccination. Required vaccination, serological testing to determine immunity, and baseline tuberculosis screening are available for all Public Health employees who elect to become DCoDPH patients upon request and at no cost.

 

Proof of vaccination and tuberculosis screening must be provided to Human Resources within seven (7) business days prior to the date of employment.
