GENERAL DESCRIPTION:

 

Performs professional and administrative work, planning needed services. Responsible for screening and evaluating youth and families who may be at risk of delinquency and/or in crisis. Work with stakeholders to integrate best practices, reduce duplication of services, ensure access to effective support, and prevent or divert from system involvement. Provide referrals and linkage to needed community services by coordinating and referring to safe, timely, effective, efficient, equitable and client centered resources in the community.

 

Work is performed under the regular supervision of the Program Manager.

 

 

DUTIES AND RESPONSIBILITIES:

 

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

 

  • Schedules and provide outreach, consultation, and interventions.
  • Conducts interviews, assessments, and screenings and develops a service plan.
  • Works with service providers and internal stakeholders.
  • Develop support systems to meet client needs by identifying and coordinating various available services.
  • Consults with program staff, outside agencies, or stakeholders’ agencies as appropriate.
  • Maintains client contact logs that indicate face to face, telephone conversations, progress notes, referrals or other contacts with clients, family members, program staff, contact agencies or other agencies.
  • Follow up with discharged clients to ensure they were satisfied with services and were still in good physical and mental health—data collection.
  • Participates in general and program staff meetings, and planning meetings; meets regularly with supervisor to discuss status, client, and related issues and to identify action and next steps.
  • Completes reports and records and other documentation as necessary and required.
  • Enter information into required databases.
  • Follow-up on client referrals.
  • Performs related work as assigned.

 

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Knowledge of Durham County and its human services agencies, churches, schools, and other organizations that provide services to adults and/or youth and their families.
  • Knowledge of case management principles and practices.
  • Knowledge of the criminal justice system in Durham County and of applicable laws and regulations.
  • Skilled in establishing rapport with clients and applying case management techniques.
  • Ability to interview clients and families to obtain information.
  • Ability to establish and maintain effective working relationships with clients, family members, community leaders, court and school officials and service providers.
  • Ability to plan and execute work plans and to coordinate work with a variety of individuals and service agencies.
  • Ability to operate computer equipment and generate reports and records.
  • Ability to express ideas clearly and concisely, orally and in written documents.

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

Bachelor’s degree in a human service field and one year experience in social work, counseling, mental health and/or case management in a public agency or school which provides experience in the techniques of casework, group work or community organization or an equivalent combination of education and experience.

 

 

SPECIAL CERTIFICATIONS AND LICENSES REQUIREMENTS:

 

  • A valid North Carolina driver’s license with an acceptable three-year driving record.

  • Spanish Speaking
