GENERAL DESCRIPTION:

 

Funding for this position ends on December 31,2026. The Case Manager will work directly with the Local Reentry Council Coordinator and program participants to address housing needs. The Local Reentry Council is an organized network of individuals and agencies that provide supervision and coordinate innovative responses to the reintegration of offenders/formerly incarcerated individuals at the local level and through the use of new and existing program services. The work performed by this position is directly related to developing a systemic approach to addressing the housing needs and barriers facing individuals how are or have been criminal justice involved. This position will serve as the primary point of contact for activities related to housing and the Reentry Housing Support project funded by the City of Durham.

 

 

DUTIES AND RESPONSIBILITIES:

  • Engage clients and establish trusting, collaborative relationships to develop plans directed toward their goal of attaining short-term or long-term housing;
  • Assist clients with developing housing goals;
  • Assist clients with assessing work history, strengths, employable skills, justice involvement, and other factors that relate to their current housing status and goals;
  • Share housing leads;
  • Cultivate relationships with local housing providers;
  • Provide assistance with the housing search;
  • Provide intensive case management to clients seeking or maintaining housing;
  • Conduct periodic home visits to assess client needs and ability to maintain housing;
  • Identify training opportunities for participants to gain skills and knowledge for employment;
  • Participate in local meetings to promote expanded/better housing options for justice involved individuals;
  • Participate in local reentry council meetings;
  • Assist with the facilitation of housing subcommittee meetings;
  • Assist with the preparation of reports for the Criminal Justice Advisory Committee  and City of Durham related to housing support;
  • Enter client information into required databases and spreadsheets to ensure accurate documentation of housing assistance;
  • Participates in general and program staff meetings, and planning meetings; meets regularly with supervisor to discuss status, client and related issues and to identify action and next steps;
  • Completes reports and records and other documentation as necessary and required;
  • Acts as chaperone for urinalysis; provides front desk coverage as needed;
  • Performs other related duties as assigned.

 

 

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Knowledge of Durham County and its housing providers, human services agencies, churches, schools, businesses and support organizations;
  • Knowledge of the continuum of housing and local housing authority;
  • Knowledge of case management principles and practices;
  • Knowledge of the criminal justice system in Durham County and of applicable laws and regulations;
  • Knowledge of local training and education providers;
  • Skilled in establishing rapport with clients and applying case management techniques;
  • Ability to interview clients and obtain factual information;
  • Ability to establish and maintain effective relationships with clients;
  • Ability to plan and coordinate work with a variety of individuals and service providers;
  • Ability to operate computer equipment and generate reports and records; and
  • Ability to express ideas clearly and concisely orally and in written documents.

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

 

Bachelors degree in a human service field and six months experience in social work and/or case management in a public agency or school which provides experience in the techniques of casework, group work or community organization or an equivalent combination of education and experience.

 

 

SPECIAL REQUIREMENT:

 

  • A valid North Carolina driver’s license with an acceptable driving record.

 

 

PREFERRED QUALIFICATIONS:

 

  • Direct services to justice involved individuals is preferred.
